A Case of Intra-abdominal Fibromatosis that Need to be Differentiated from Pancreatic Malignancy

Abstract
Intra-abdominal fibromatosis is a rare monoclonal, fibroblastic proliferative tumor arising in abdominal musculoaponeurotic structure. The majority of fibromatosis are associated with familial adenomatous polyposis of colon and Gardner¡¯s syndrome. However, sporadic forms are very rare manifestation. We report a case of a 40-year-old male who was diagnosed as an intra -abdominal fibromatosis incidentally. We performed biliary pancreas CT, pancreas MRI, EUS-guided FNA for incidentally detected intra-abdominal tumor that was attached to the pancreas. The size of tumor was increased and cytology showed atypical cells, the possibility of pancreatic malignancy was considered. So we decided the operation to remove this lesion and confirm pathologic diagnosis. Central pancreatectomy and pancreato- jejunostomy was performed and pathologic diagnosis was intra-abdominal fibromatosis. Treatment of choice for fibromatosis is surgical resection, however, free resection margin is difficult to get enough. Therefore, close monitoring is important
because of high recurrence rate due to remnant tumor cells.
